High CPAP vs. NIPPV:
A physiological cross-over study

Little is known about the relative impact of High CPAP (≥9 cmH2O) vs NIPPV on physiological parameters. In this crossover study, we placed infants who were already on either High CPAP or NIPPV to the other mode at the same mean airway pressure. We evaluated left and right ventricular cardiac output and work of breathing (using electrical diaphragmatic activity as a surrogate). We then placed babies back on the original mode and measured the same outcomes.

We found no difference in either cardiac output or work of breathing measures, supporting our hypothesis that high CPAP may not lead to any detrimental physiological impact compared to NIPPV.
